Switch to Direct Deposit

CTPF advises all members who receive benefit checks to sign up for direct deposit. To initiate a direct deposit, you must complete and return CTPF Form 425 with the required documentation. Forms should be submitted by fax 312.641.7185 or email an attachment (.pdf file) to imaging@ctpf.org. Direct deposit is the best way to ensure timely delivery of pension payments. 

Mail Order Pharmacy Options

Delayed mail service may impact members who receive prescriptions through a mail-order pharmacy program. Members may wish to switch to a local retail pharmacy if you are experiencing mail disruptions. Typically, the plan can provide a  pharmacist-to-pharmacist transfer of a prescription to a retail pharmacy. The plan may also be able to arrange for an emergency supply through the retail pharmacy until the mail service delivery arrives.

Express Scripts

The AARP® Medicare Supplement Insurance Plan F (UnitedHealthcare) and  UnitedHealthcare Group Medicare Advantage PPO for CTPF utilize the Express Scripts pharmacy program. If you have coverage in one of these plans, call the Express Scripts Customer Service Department, 1.800.864.1416, or visit  www.Express-Scripts.com/medd/ctpf for additional information. CTPF’s group number is CTPFRX.

BCBS, UHC, and Humana Plans

If you have prescription drug coverage through another CTPF plan, call the pharmacy program customer service number on the back of your medical plan ID card for additional information. 

Reminder Send Forms and Documents by Fax or Email

For the health, safety, and wellbeing of our members and staff, the CTPF office remains closed to the public, and staff are working remotely. During our remote operations, U.S. Mail processing may be delayed. The best way to ensure timely delivery is to send benefit applications, forms, and documents by fax 312.641.7185 or email an attachment (.pdf or .jpg format) to imaging@ctpf.org.
